Consents for OSHC and Vacation Care
As per enrolment form (Consent Required Prior to Child/ren beginning care – Please ensure you have read
carefully and checked the Yes/No Box on the enrolment form)
1/ Do you consent for the service to seek medical treatment for your child from a medical practitioner,
hospital or ambulance in the event you cannot be contacted?
2/ Do you consent for you child to be transported by an ambulance service?
3/ Do you consent for the service to take photographs and videos of your child during normal activities and
excursions.
4/ Do you consent to provide you child with a sun safe hat for outdoor activities?
5/ I give permission for educators with current first aid to administer paracetamol in an emergency in the
correct dosage for the age of my child. Administration of this medical will only be given in the event of a
parent being un-contactable in consultation with the director or nominated supervisor.
6/ I consent for the service to administer Ventolin or Epi-pen to my child in case of emergency.
7/ I agree to accurately record the arrival and departure of my child from the service in accordance with the
service requirements.
8/ I give permission for educators and school teachers/principal to share information about my child in
relation to their care and wellbeing plan.
9/ I give permissions for my child to use/view technology (i.e. Tablet, TV, Ipod)
10/ I give permission for child’s photo to be displayed in public places.
11/ I give permission for my child to participate in regular local excursions from the service by foot. (Regular
outing excursions forms will be signed by parents/guardians at the service.
12/ I agree to notify the service when my child is to be collected by any person other than those listed on the
enrolment form in accordance with the services and procedures.
DS Sports Terms and Conditions (Before and After Care)
1/ I confirm that I have provided DS Sports with all details relating to my child. This includes court orders,
health and medical conditions, dietary requirements and medications.
Please note: Where children with a disability or medical condition enrol and it is determined that
unique or specific training is required to ensure adequate care can be provided, bookings cannot be
made for 4 weeks to allow for training and applications for additional staff if required.
2/ A late fee of $15.00 per 10 minute block or part thereof will be incurred by the family. DS Sports is not
licensed to have children on the premises before or after hours of operation. This is a breach in the
Education and Care Regulations.
3/ If my child is not attending a booked session I will notify DS Sports as soon as practically possible. If my
child does not arrive at after school care for a booked session staff will follow the procedure outlined in the
services risk assessment. This includes contacting the parent/guardians, the school and then the police.
4/ I acknowledge and agree that subject to law DS Sports is not liable for personal injury, property loss or
damage sustained by any child whilst attending any DS Sports program. (except where there is a direct
consequence of a DS Sports staff member).
5/ I acknowledge that DS Sports has a policy and procedure in relation to behaviour guidance. Staff will
follow these procedures. If a child continues to demonstrate disruptive, violent or otherwise inappropriate
behaviour DS Sports will contact me to collect my child. A behaviour plan with strategies appropriate for my
child’s age and development will then be implemented. In circumstances where a child continues to display
disruptive, violent or otherwise inappropriate behaviour DS Sports will review the child’s enrolment.
6/ A hard copy of all DS Sports policies and procedures are available for perusal at each service. I understand
that if I request a copy this needs to done through a manager.
7/ If I require last minute care for my children my booking will only be accepted if
DS Sports has the capacity the accept my booking.
Ratio’s at the service can be met.
The child has been previously enrolled at DS Sports.
8/ I acknowledge that all information provided to centrelink is true and accurate and is the same information
provided to DS Sports. In the event of any discrepancies DS Sports are not responsible for any fees
outstanding and these fees must be paid until all required and correct information has been provided to
centrelink.
9/ I understand that in the event of an extended session of after school care, such as the last day of term
there may be an increased fee for these sessions.
10/ I understand that in the event I wish to cancel a booking for my child’s before or after care I can do so
with more than 48 hours notice and there will be no charge to me, if I am wishing to cancel a booking within
the 48 hours time frame there will be a charge of $10 to me, and if I am to cancel the booking on the day I
will be charged the full session amount (less any Child Care Subsidy).
11/ I have read the terms and conditions set by Xplor regarding my payments to DS Sport. I understand the
fees are charges set out for the Direct Debit. There will be a charge of $20 for any failed transactions from
my nominated account.
12/ I understand that in the even I wish to ca
ncel a booking for my child’s vacation care with less than 2
weeks notice I will be charged full fee (less any applicable Child Care Subsidy) for these sessions. Please
understand that our staff rostering, planning for each day including booking buses and purchasing resources
for special events at each vacation care is finalised before the program begins.
13/ I give permission for my child/ren to participate in all enrolled program activities. I acknowledge that
activities may include watching G rated videos/movies. Families will be informed prior to the day if any
videos/movies are being watched.
